Gateway Healthcare is hiring Case Workers on our Outreach teams to work with adults managing mental health issues, including substance abuse & co-occurring disorders.  This is a community-based position that will require travel statewide.Â

Responsibilities:Â

• Provide case management, rehabilitation, crisis interventions, and outreach services in home, office, and community settings to adults with mental illness
• Collaborate with psychiatric & allied mental health teams in assessing diagnosis intervention & treatment planning
• Conduct substance abuse treatment with clients
• Provide clinical interventions & linking Clients with community resources, such as accessing medical & dental care, affordable housing, financial benefits, developing support networks, and applying ADL skills in needed real-life environments
• Conduct comprehensive clinical assessments & develop interventions for substance abuse, abuse, symptom management, and family support & education
• Coordinate emergency psychiatric services within a timely manner, including documenting interventions, alternatives considered, and response of Client/Client’s Family, and supporting agencies
• Participate in treatment & discharge planning

Requirements:

• Associate’s degree, required.  Bachelor’s degree, preferred.Â
• Experience working with adults with mental illness, substance abuse, trauma/abuse histories, or involvement with the criminal justice system, preferred
• Valid driver’s license & insured vehicle

Shift & Schedule:  M-F 8am-4:30pm (Full-Time, 40 hours/week)

Location:ÂOpenings available in Pawtucket, RI or Johnston, RI or Charlestown, RI

Compensation:  Compensation starts at $19.00/h and is commensurate with experience
