Responsibilities:
- Responsible for the clinical & administrative oversight, supervision, and support to an adult’s Assertive Community Treatment (ACT) team that provides community-based services & supports to adults residing in the community who live with a severe & persistent mental illness
- Assess clients to determine program eligibility, engagement in treatment, appropriate level of care, coordination of care, and discharge planning
- Assign client caseloads & ensure all clients receive care in accordance with individualized treatment plans.
- Mentor staff in providing education & support to clients & their families, regarding diagnosis, treatment options, recommendations, and self-care techniques
- Ensure coordination of client care with practitioners, including Psychiatrist, Primary Care Physician, and other medial or clinical practitioners
- Provide daily, ongoing supervision, support, and performance improvements to staff
- Assist staff to identify client & family needs and make appropriate referrals as needed
- Assist with outreach services as needed, including conducting risk assessments, and implementing crisis intervention strategies/safety planning to maintain client safety, and escalate care as needed
- Ensure the collection of required data for BHDDH & Gateway & monitor program budgets
- Provide individual & group supervision to team members & maintain supervision records in accordance to BHDDH regulations & Gateway policies & procedures
Requirements:
- Master’s degree in related field, or RN. Licensed independent practitioner (LICSW, LMHC, LMFT), preferred
- Prior experience supervising & mentoring staff
- Demonstrated expertise with adults with behavioral health needs
- Knowledge of community resources for clients with behavioral health/substance use disorders
- Valid driver’s license & use of car with liability insurance coverage, or other means of transportation in order to perform the major functions of the job
Shift & Schedule: M-F 8am-4:30pm (Full-Time, 40 hours/week)
Location: Johnston, RI
Compensation: Compensations starts at $61,000 and is commensurate with experience
