Locations: Cranston
- Duties and Responsibilities:
- Provide comprehensive case management, direct assistance, and advocacy to assist participants in obtaining basic necessities of daily life (i.e. food stamps, home energy assistance, transportation, housing, legal advocacy for patients involved in the criminal justice system)
- Collaborate in the treatment process and participation in care coordination with the multidisciplinary treatment team
- Assist patients with coordinating meetings, appointments, and telephone conversations with outside resources and agencies within the local community support programs
- Coordinate, schedule, and administer the agency’s assessment of clients’ health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate substance use treatment with medical treatment
- Other duties as required
Qualifications:
- Bachelors Degree and minimum of 1 year experience in the field of addictions preferred
