Location: Providence

MAJOR DUTIES AND RESPONSIBILITIES:

  • In collaboration with the team physician and Health Home Team, coordinate schedule and administer agency’s assessment of clients’ health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate substance abuse treatment with medical treatment. (comprehensive care management)
  • Coordinate on-going training to case managers in the recognition and management of chronic medical conditions (health promotion).
  • Coordinate and integrate disease self-management activities (improve integration within general health promotion practices) (care coordination).
  • Coordinate effective discharge planning implementation/continuity of care (care coordination, comprehensive transitional care).
  • Ensure quality communication with CMHO’s, federally qualified health centers, hospitals and other providers (troubleshoot when necessary) (care coordination).
  • Develop and maintain working relationships with primary and specialty care providers and inpatient facilities (comprehensive care management).
  • Consult with community agencies and families to maintain coordination of the treatment process (referral to community and social support services, individual and family support services).
  • Assure that team is meeting overall Health Home goals (comprehensive care management) including documentation and utilization.
  • Educate community health referral sources; perform clinical screens and participating, organizing and executing care-coordination and prevention (health promotion).
  • Design and develop prevention and wellness initiatives (health promotion).
  • Monitor Health Home performance and lead improvement efforts (care coordination).
  • Obtain and sustain Motivational Interviewing proficiency.
  • Perform other duties as required
  • Patient screening, intakes, assessment, treatment planning, counseling, case management, crisis intervention, patient education, facilitate referrals, discharge planning, follow-up after care and record-keeping, orientation, consultation and relapse prevention
  • Demonstrate understanding of and adherence to appropriate ethical behavior as stipulated in theCODAC Policy and Procedures Manual
  • Demonstrate understanding of and adherence to confidentiality regulations as stipulated in the CODAC Policy and Procedures Manual, Federal Regulation 42CFR part 2 and other pertinent Federal and State laws and regulations
  • Maintain individual and group caseloads in accordance with CODAC procedures
  • Monitor urine and breathalyzer tests when necessary
  • HIV pre/post test counseling
  • Speaking engagements when required
  • Obtain and/or maintain appropriate credentialing and licensure
  • Provide basic office tasks, if necessary
  • Provide case presentations at clinical meetings
  • Document all services, in the patient record, to reflect Health Home and Treatment Services
  • Participate in Performance Improvement activities

QUALIFICATIONS: Bachelor’s Degree required, LCDP required

Pay: From $22.00 per hour
