This is a half-time (20 hours/week) position responsible for the launch and overall administration and project management of the new Go Team School Violence Prevention Project funded by the Office for Juvenile Justice and Delinquency Prevention. This project involves a partnership between FSRI, the Central Falls School District and the Central Falls Police Department WHO WILL COLLECTIVELY identify and support children and youth at high risk for youth violence, victimization, and/or family disruption in order to prevent/reduce school violence in all Central Falls public schools (elementary through high schools), improve school safety, and prevent youth violence and victimization.

Provides coaching, support, supervision and education during parental visits with children who are placed out of the home. Coordinates and maintains schedule of coaching and visitation. Responsible for reports to and communication with DCYF including letters for court approved by the Clinical Director. Coordinate with families/caregivers providers to best support treatment work. Provide case management. Provide transportation for children to and from visits as needed.

Provides trauma-focused diagnostic assessments and enhanced evidence-based services for children, youth and their caregivers in the community and in the office. Collaborates on cases with case managers, psychiatric team, and other service providers to ensure best client care. Provides emergency crisis intervention/assessment as needed. Works as part of a trauma-focused multidisciplinary team to implement evidence-based practices

The Integrated Health Home (IHH) team is a multidisciplinary team offering psychiatry, nursing, case management, peer recovery support, hospital liaison, and housing support. The primary focus for this position is care coordination and addressing the social determinants of health with the intention of improving health and well-being.
