Elaine Houghtaling and her daughter Nicole proudly carry forward the incredible legacy of their beloved husband and father through the Bob Houghtaling Foundation.
With the assistance of the workshop facilitator, attendee's will complete the CAQH (Provider Data Portal) application as this is the portal all insurance companies use in their credentialing process. Attendees will also complete the state Medicaid/Medicare application and United Healthcare application. By the end of this workshop, attendees will have completed applications for the above insurances/& data portal.
