MAJOR DUTIES AND RESPONSIBILITIES: In collaboration with the team physician and Health Home Team, coordinate schedule and administer agency’s assessment of clients’ health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate substance abuse treatment with medical treatment. (comprehensive care management) Coordinate on-going training to case managers in the recognition and management of chronic medical conditions (health promotion). Coordinate and integrate disease self-management activities (improve integration within general health promotion practices) (care coordination). Coordinate effective discharge planning implementation/continuity of care (care coordination, comprehensive transitional care). Ensure quality communication with CMHO’s, federally qualified health centers, hospitals and other providers (troubleshoot when necessary) (care coordination). Develop and maintain working relationships with primary and specialty care providers and inpatient facilities (comprehensive care management).

Locations: Cranston Duties and Responsibilities: Provide comprehensive case management, direct assistance, and advocacy to assist participants in obtaining basic necessities of daily life (i.e. food stamps, home energy assistance, transportation, housing, legal advocacy for patients involved in the criminal justice system) Collaborate in the treatment process and participation in care coordination with the multidisciplinary treatment team Assist patients with coordinating meetings, appointments, and telephone conversations with outside resources and agencies within the local community support programs Coordinate, schedule, and administer the agency’s assessment of clients’ health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate substance use treatment with medical treatment Other duties as required

This position supports survivors of domestic/interpersonal violence and sexual assault (D-IPV/SA) and their families on their journey toward housing security, healing, and self-sufficiency. Provides housing information, referrals, advocacy, and support in obtaining and maintaining safe and stable housing. Provides coaching, education and triage in tandem with Survivor Support Program Victim Advocate.

To provide guidance and supervision for 5 youth 16-21 who identify as part of the LGBTQ+ community. To assist each resident to make decisions about daily life, conduct and discipline within the boundaries of agency policy and the requirements of the contractual agreement with the Department of Children, Youth and Family.

Responsible for providing on-scene crisis intervention, emergency screening and triage, information, emotional support, language support, and referral in collaboration with law enforcement to individuals, children/youth and families exposed to victimization and trauma in the community. Provide appropriate follow up support and resources for assistance with the criminal justice system, trauma treatment, and access to basic needs, treatment, and government benefits.

Responsible for providing home/community-based case management and/or behavioral support services to clients and their families receiving evidence-based services such as Dialectical Behavior Therapy. Advocates for the clients and families to access appropriate and necessary resources from Human Services and similar departments/agencies to improve the quality of their lives. Facilitates group skills sessions for youth ages 8 to 18.

This position has direct oversight of day to day activities of the permanent supportive housing program including safety protocols, client participation, grant implementation and quality management, staff oversite, and program development. This position also interacts directly with landlords, property managers, and developers, and provides support within the landlord/tenant relationship. This position is required to be on the membership of the RI Continuum of Care
