Our Community Support Program/ Wellness and Recovery Services is seeking a Part-Time Peer Recovery Specialists to provide a bridge between providers and adults with serious mental illness. Peer Specialists will assist in facilitating the care and well-being of adults receiving services while working on a multidisciplinary team. Peer Recovery Specialists would have lived experience with behavioral health issues and/or addiction, possess the ability to work and communicate effectively, and have an outgoing and engaging personality to connect with individuals. The position requires one to share lived experience of mental illness and/or addiction (when appropriate), serve as a role model for peers, and share self-help techniques and effective coping strategies. Peer Recovery Specialists will help peers develop natural support systems.

Safe Haven offers a set of supportive services for individuals who are unhoused and actively using substances, often with co-occurring mental health challenges. MAJOR DUTIES AND RESPONSIBILITIES: In collaboration with the team physician and Health Home Team, coordinate schedule and administer agency’s assessment of clients’ health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate substance abuse treatment with medical treatment. (comprehensive care management) Coordinate on-going training to case managers in the recognition and management of chronic medical conditions (health promotion). Coordinate and integrate disease self-management activities (improve integration within general health promotion practices) (care coordination). Coordinate effective discharge planning implementation/continuity of care (care coordination, comprehensive transitional care). Ensure quality communication with CMHO’s, federally qualified health centers, hospitals and other providers (troubleshoot when necessary) (care coordination). This position supports survivors of domestic/interpersonal violence and sexual assault (D-IPV/SA) and their families on their journey toward housing security, healing, and self-sufficiency. Provides housing information, referrals, advocacy, and support in obtaining and maintaining safe and stable housing. Provides coaching, education and triage in tandem with Survivor Support Program Victim Advocate.

To provide guidance and supervision for 5 youth 16-21 who identify as part of the LGBTQ+ community. To assist each resident to make decisions about daily life, conduct and discipline within the boundaries of agency policy and the requirements of the contractual agreement with the Department of Children, Youth and Family.

Responsible for providing on-scene crisis intervention, emergency screening and triage, information, emotional support, language support, and referral in collaboration with law enforcement to individuals, children/youth and families exposed to victimization and trauma in the community. Provide appropriate follow up support and resources for assistance with the criminal justice system, trauma treatment, and access to basic needs, treatment, and government benefits.

Responsible for providing home/community-based case management and/or behavioral support services to clients and their families receiving evidence-based services such as Dialectical Behavior Therapy. Advocates for the clients and families to access appropriate and necessary resources from Human Services and similar departments/agencies to improve the quality of their lives. Facilitates group skills sessions for youth ages 8 to 18.

This position has direct oversight of day to day activities of the permanent supportive housing program including safety protocols, client participation, grant implementation and quality management, staff oversite, and program development. This position also interacts directly with landlords, property managers, and developers, and provides support within the landlord/tenant relationship. This position is required to be on the membership of the RI Continuum of Care
