    Patient Acuity Level Guideline

    Course outline · Module 5

To ensure we can continue providing individualized services, the patient acuity level guideline below was created. Please note that it serves as a guideline only and is not an official document. There is flexibility within the three levels when assigning patient acuity.

For example, a client with diabetes may be hospitalized because of blood sugar issues. Upon discharge, they might need additional services and temporarily move from Level 3 to Level 2. Once stabilized, they would return to Level 3.

Teams were initially based on 125 clients. Because of the levels, we were able to manage up to 160 clients per team, depending on the distribution across levels. Team coordinators are responsible for ensuring that no single team is made up of clients from only one level.
